چکیده
A 61-year-old male presented to the emergency department (ED) with painless diplopia, left-ptosis, and left downward gaze, 3 days after sustaining a fall from standing height with subsequent lumbar and head trauma. Prior to the ED consult, his only symptom was persistent generalized high intensity headache. On physical examination, no other neurological deficit was found. Computed tomography (CT) scan showed Fisher 4 subarachnoid hemorrhage (SAH). Cerebral angiogram and brain magnetic resonance imaging (MRI) were negative. Screening for possible secondary causes of isolated third-nerve palsy (TNP) were all negative. To our knowledge, this is the first report of a traumatic SAH with delayed onset of an isolated complete TNP as its manifestation. CONFLICTS OF INTEREST/DISCLOSURES None pertinent to this research. AUTHOR JUSTIFICATIONS All authors have provided original or professional content and were involved in the clinical care of the patient. Ramsay Hunt Syndrome Associated with True Vocal Cord Palsy- A Case Report
Introduction: Varicella-zoster virus may cause an infectious disease called Ramsay Hunt syndrome. The related symptoms include facial nerve palsy (FNP), otalgia, the vesicular eruptions of the auricle and external auditory canal, less common ocular movement disorder, facial hypoesthesia, myofascial pain, vestibular symptoms, hearing loss, dysphasia, vocal cord paralysis, as well as tongue paral...
متن کاملIncomplete oculomotor nerve palsy in the subarachnoid space caused by traumatic brain injury.
A patient with traumatic brain injury showed incomplete oculomotor nerve palsy in the subarachnoid space. A 12-year-old girl was hospitalized after a head injury. Neuro-ophthalmic examination showed that the left eye had a ptosis and pupillary involvement. An MRI indicated an intracranial hematoma at the basilar portion of the left temple. The ptosis and pupillary involvement improved after eli...
متن کاملAn Unusual Complication of Hypertensive Hemorrhage – Delayed Oculomotor Palsy: Case Report and Literature Review
We present a case of oculomotor palsy due to hypertensive hemorrhage in the caudate nucleus, with intraventricular extension. To our knowledge, this is the only instance of this complication occurring due to hypertensive hemorrhage. Our patient initially developed headache at the time of her hemorrhage; 8 days later, she developed complete third nerve palsy, which showed improvement at follow-u...
متن کاملECG changes of cardiac origin in elderly patients with traumatic brain injury
Background: Simultaneous electrocardiographic (ECG) changes are seen in hemorrhagic brain events even in the absence of associated myocardial infarction (MI). This study was designed to assess the role of ECG changes to predict true MI in patients with hemorrhagic brain trauma. Methods: Data of 153 patients with traumatic brain injury and concomitant ECG changes were recorded. Enzyme study w...
متن کاملDelayed Rebleeding of Cerebral Aneurysm Misdiagnosed as Traumatic Subarachnoid Hemorrhage
An intracranial saccular aneurysm is uncommonly diagnosed in a patient with closed head trauma. We herein present a patient with delayed rebleeding of a cerebral aneurysm misdiagnosed as traumatic subarachnoid hemorrhage (SAH). A 26-year-old female visited our emergency department because of headache after a motorcycle accident. Brain computed tomography (CT) showed a right-side dominant SAH in...
